Job Purpose: In the Community Habilitator position you will provide support services to teenagers and adults with developmental disabilities.  These services shall include the promotion of continuous learning and individual growth opportunities within the community.  Opportunities shall be based upon the participants’ interest areas.

Responsibilities include, but are not limited to:

  1. Meet individuals in their home and in the community/identified on the individual’s Community Habilitation plan.
  2. Implementation of the individuals’ habilitation plan.
  3. Provide meaningful and functional activity-based programming consistent with individual program/support plans.
  4. Serve as an appropriate role model and engage in interactions which promote and insure the dignity and rights of the individual.
  5. Accurately communicate and complete all documentation for individual program plans, reportable events and incidents within established periods.
  6. Ensure necessary resources are available as needed to complete scheduled activities.

Job Purpose:  In the Lead Caregiver position in our Wrap Around After School Program you will work with children and adults who have developmental disabilities in an afternoon respite program.  You will participate in daily activities of the program, supervise the participants, and implement recreational activities.


Responsibilities include, but are not limited to:

  1. Provide personal care to participants as needed.
  2. Work with other staff in a cooperative, team environment.
  3. Impart the agency’s philosophy of “Respect, Integrity, Service and Excellence” to individuals, families, program staff and community resources.
  4. Maintain confidentiality of information relating to individuals and their families in accordance with agency policy.
  5. Adhere to the agency’s policies and procedures as well as department guidelines.
  6. Perform other duties at the discretion of the Program Director or his/her designee.

Hours of Employment:

Full-Time 36 hours/week:   Mon & Tue 12:00pm-6:00pm

                                               Wed & Thu 10:00am-6:00pm

                                               Fri 11:00am-6:00pm
  1. High school diploma or GED.
  2. Valid and clean NYS Driver License required
  3. Associates degree in Human Services or related field preferred.
  4. 1 year experience providing service to individuals with developmental disabilities.
  5. Candidate must have demonstrated ability to plan and organize daily activities of staff and participants.
  6. Ability to communicate effectively, both orally and in writing.
  7. Excellent leadership and motivational skills.
  8. Able to meet for bi-weekly supervisor meetings.
  9. Ability to meet all agency training requirements, including CPR and First Aid certifications.
  10. Sitting, standing, walking, lifting up to 50 pounds, pulling and pushing wheelchairs and adaptive equipment, carrying, grasping, reaching, stooping and crouching, and crawling may be required.
