Job Purpose:

In the Medical Liaison position you will support adults with intellectual & developmental disabilities.

Hours of Employment: Full Time 40 hours per week ~ Monday - Friday 8:00 am - 4:30 pm.
Key Job Skills:
  • Ensuring all individual medical/health needs are addressed promptly under the clinical direction of the Registered Nurse.
  • Serving as a liaison between CP Rochester and external service providers.
  • Providing evaluation of individuals' illness or injury and consulting with the RN for follow up.
  • Scheduling, transporting, and accompanying individuals, with all appropriate medical information, to all routine medical, psychological, therapy, interim or emgergency appointments as assigned and acts under the direction of the RN.
  • Communicating all appropriate information to Residential RN, service providers, families, individuals, and service coordinator.
  • Ensuring all medications, treatments, first aid supplies, and adaptive equipment are available and are administered as prescribed and are maintained within expiration dates.
  • Reviewing Medication Administration Records weekly as assigned by the RN and bringing any issues to the attention of the RN in a timely fashion, specifically those concerning frequent use of prn medication and inappropriate medication practices.
  • Ensuring that accurate weights and vital statistics, as indicated in the individual's care plan, are obtained for each individual and documented.
  • Securing documentation of all appointments on the appropriate form. Completing related documentation and communication as assigned.
  • Implementing treatment plan and regimens as prescribed by the physician or health care provider.
  • Monitoring and assuring that established infection control procedures are followed.
Essential Functions: Physical Requirements include: sitting, standing, walking, lifting over 50 pounds, pulling and pushing wheelchairs and adaptive equipment, carrying, grasping, reaching, stooping and crouching, crawling, color determination, visual acuity, speaking clearly, listening to spoken communication, sense of taste and smell.
  • High School Diploma or GED required. 
  • Experience working with people with developmental disabilities strongly preferred.
  • Valid and clean New York State Driver’s License.
  • Demonstrated ability to effectively communicate orally and in writing.
  • Ability to complete required training courses included but not limited to First Aid, CPR and Medication Administration.
