CP Rochester is proud to offer a large depth of knowledge on community services and supports available to individuals and their families. Our Service Coordinators will develop and implement an Individualized Service Plan. This plan reflects an individual’s personal goals and desires. Our Service Coordinators will also provide advocacy and support personal choice so that individuals can achieve greater independence in their community.
The following are funding sources for our Service Coordination Department:
- Home and Community Based Services Waiver (HCBS):OPWDD funding- Medicaid Service Coordination (MSC) and Plan of Care Support Services (PCSS)
- Care at Home Waiver: OPWDD Funding
- Nursing Home Transition and Diversion Waiver (NHTD): DOH funding
- Traumatic Brain Injury Waiver (TBI): DOH funding
